Lower Grafton / Sullivan County

  • RPC Conducting food mapping and policy audits. Results TBD - intended to impact long-term development

North Conway, NH

  • Established a farm to table standard. Offering organic, locally grown food through the collaboration between local farms and restaurants

Strafford, NH

  • Worked with local land owners to conserve land for recreation and watershed protection.

Plymouth, NH

  • ELFS - Eco-learning Farm-stead project: collaborative food gardening project to bring children and adults closer to foods and how to grow them.

Bristol, NH

  • Received several rounds of SRTS Grants

Newfound Schools, NH

  • Working on making classroom celebrations non-food related events.

Lebanon & Mascoma, NH

  • New trails sections opened and cleared. Results: more youth choosing P.E. in classes focusing on outdoor pursuits using trails.
  • Implemented Reach the Peak Healthy Lunch program. Result: 350-400 youth and their parents challenged themselves to eat more fruits and veggies at lunch.

Lebanon, NH

  • Implemented NAP SACC in 5 early care centers: Result: sustainable improvements in food programs / activity levels.
  • BMI Screening, and 5-2-1-0 embedded in multiple medical practices
  • Safe routes to School resulted in 75-100 youth walking to school 1-2 days a week.
  • Implemented EBT / Debit capacity at Lebanon Farmers Market. Result: Increased sales at market
  • Implemented peer-based healthy foods marketing program to drive students to full / complete meals
  • Developed community garden on contributed NPO-owned land. Results: 20+ families access garden plots. Children's garden accessible to nearby child care.

Mascoma, NH

  • Linked multiple partners to evolve volunteer-led running program at Indian River Middle School. Result: 29 students participating in fall running
  • Piloting NAP SACC in small family home day care Fall 2011. 6 providers enrolled in the pilot study
  • Policy change to limit single item calories in ala carte menu at school. Result 50% reduction in junk food calories sold.

Dartmouth Hitchcock Medical Center

  • Eliminated deep-fried food in cafeteria. Result: More people purchasing higher nutrition meals, revenues increased.
  • Tina Layton, RD Educated all pediatric nurses on 5-2-1-0 and basic nutrition so they can discuss with patients

Franklin, NH / Odell Park Committee

  • Installed 4 exercise stations in ½ mile, new playground equipment, new gazebo and new fence.

Bristol, NH / TTCC

  • Implemented CATCH in After School Program

Londonderry, NH

  • Repair of bridge by volunteers at Londonderry Trails. Maintenance of trails.
  • Director for Food Services for school district is trying to make changes in the food served in our schools after taking a healthy eating and living class.

Manchester, NH - Full Spectrum Wellness

  • Introduced the Class Pass Program offering individuals that have been laid off or are unemployed 50% discount off of attending wellness classes and workshops (i.e. Yoga, meditation, stress management, mindful eating) recognizing the need to maintain healthy and positive outlook.

Bedford, NH

  • Expanded Pulpit Rock open space and extended walking trails.
  • Building community park with ice rink

Concord, NH

  • Organized a community coalition for the capital region and started an assessment of needs and assets

Monadnock Region, NH

  • New Monadnock community market coop to be built by summer 2012 partially funded by member loaning.
  • Promotion of breastfeeding task force
  • Baby friendly hospital designation work
  • Keene State led research
  • Community action plan with worksite policy initiatives for continuation
  • Keene community kitchen serving hot meals with significant degree of local or organic sourcing, also is one of our Turn 4 New Leaf dining locations.

Goffstown

  • Food Pantry - offering cheese, milk and eggs instead of store gift cards
  • Cross country skiing on track loop
  • Lacrosse as new sports program
  • Includes healthy recipe each week on church newsletter.
  • Implemented SHINE - activity and healthy eating for seniors.
  • Started a preschool playgroup
  • Opened a produce stand

StayNHealthy, 5-2-1-0

  • Lila Monahan, MD from Foundation Medical Partners - a 2-year learning collaborative to provide tools and community resources to medical providers to address obesity in the doctor's office

Nashua, NH

  • Implemented Nashua Steps into Health - a city-wide spring walking program
  • Implemented FFVP: Fresh Fruits and Veggies Program

  • Planted the seed for MWV Green Team who has managed community garden for 3 years.
  • Serve as advisor for Evergreen Institute for Wellness - 2012 theme: FOOD
  • Senior Farmers Market nutrition program statewide
  • Volunteered for my school district's wellness committee.
  • Offering a pilot initiative in BC/AC which includes teaching local/organic food eating and preparing with CATCH physical activity
  • Initiations of worksite wellness in 5 companies

 

  • Applied for and received funding from FHA to build footbridge over river in my community
  • Raised funds for local police department to purchase a roadside / portable speed monitor
  • Tested the NH LWC toolkit
  • Worked with town officials and RPC on a NH DOT Community Sensitive Solutions process.
